A public community system offering early detection, monitoring and community action for people in a situation of loneliness and/or fragility.
The system targets:
A public community system for early detection, monitoring and decision-making for elderly people in a situation of loneliness and/or fragility.
This is a combined system of data science, community activation and psychosocial services. The system guarantees the universality of community supervision systems, early detection of situations of vulnerability, and risk level classification. All this assists with decision-making in relation to intervention in emergency situations, and also in the development of specific community and socioeducational intervention services and programmes.
