Objetives:

Reduce errors in taking medication, lowering the risk of a deterioration in patients’ health and premature hospitalisation.
Savings for the health system thanks to better maintenance of treatment. The WHO has drawn up an estimate of the premature deaths and health costs associated with failure to maintain treatment.
With no systems in place to assist polymedicated patients, maintenance of treatment falls short of 50%. Systems to organise and help patients take their medication can achieve 95%.

Service for chronic polymedicated patients, based on a weekly medication blister pack along with a number of technological solutions such as a hand-held device. Smart Home Care has a warning system on WhatsApp, helping the patient to take their medication and helping carers to supervise, thus ensuring full continuation of the treatment prescribed.

Warning systems to remind patients and help them take the medication wherever they may be.
With backup from the patient’s care network using simple, effective communication systems.
The treatment continuation report will be available for patients and their care networks.
